Where you can attend a video visit

A Maryland licence lets your clinician treat you while you are physically in the state. Join from a private room at home, at work or anywhere else in Maryland. You do not need to be anywhere near Baltimore. The requirement is only that you are inside the state when the visit begins.

A clinician may diagnose chronic insomnia when sleep is hard three or more nights a week and has been for at least three months. That includes trouble falling asleep, staying asleep or getting back to sleep. Cognitive Behavioral Therapy for Insomnia (CBT-I) is a short course of weekly sessions that retrains when and how you sleep without medication, and it is usually the first treatment recommended for this pattern.

Insurance treats CBT-I as a therapy visit. Our staff checks your coverage before treatment starts and gives you an estimate of what you would pay. Paying without insurance is also possible.

What treatment involves

The first visit lasts about 50 minutes. You describe how you sleep now, how long the problem has run and what you have tried, and your clinician checks whether insomnia is the whole picture. After that, you keep a sleep diary: two minutes each morning recording when you fell asleep, when you woke and when you got up. The diary shapes your sleep window, a set bedtime and wake time that moves as your sleep steadies.

Treatment commonly runs six to eight visits, with one visit in most weeks, and some people see changes in the first few weeks. Pauses in nighttime breathing can signal sleep apnea, which can occur alongside insomnia. We treat the insomnia, partners treat the apnea, and your clinician coordinates any needed referral. A video visit needs no special equipment. A phone, tablet or computer with a steady connection works, and you want a place where you can speak privately. Before the first appointment, have your medicine list and recent sleep times ready. Your clinician explains the diary and the schedule during the visit.

Which insurance plans do you accept?

Across Maryland, we currently bill Aetna, CareFirst BlueCross BlueShield, Cigna / Evernorth, Medicare, UnitedHealthcare / Optum. Before the first visit, our staff reviews your plan and explains the expected charge. Self-pay is another option.

Can I join a session while I am in Washington, DC?

Only while you are in Maryland. Where you are sitting during the appointment is what counts, wherever your home is. Any private place inside the state works. If you will be in the District or another state, choose a different appointment time.

How do I know whether I need an insomnia specialist or a sleep study?

A sleep study is an overnight test that looks at breathing and movement. Insomnia is usually identified from your history and your sleep pattern. Mention loud snoring, gasping or stopped breathing, since sleep apnea and insomnia often occur together. We treat the insomnia, we have partners who treat the apnea, and our clinician coordinates any referral that is needed.
